2. Is Ryouma's partner the actual Psychemon or a reformed Quartzmon?

1 - Potentially!

2 - Actually the next parts are going to answer that question. But you want to know, Quartzmon never meant to exist, it wasn't a digimon for a start, it was just a bunch of data that gained life after Bagramon invaded the real world. It wasn't possible for it to revive if it gained it creepy form in cost of Astamon's life.
